Terms and Conditions

The Proposed Work is to be paid in full at the time service is rendered by cash, check, or credit card unless other arrangements have been made in advance.

Returned checks are subject to a $30.00 service fee in addition to the amount of the invoice plus any collection fees if they become necessary including a 1 1/2% per month (18% annual) interest rate on unpaid balances after ten (10) days from date of work order / invoice.

All materials and parts furnished in the Proposed Work remain the property of Hirth Plumbing & Heating Company until payment in full has been received and processed for settlement through proper channels with the transfer of funds in full payment of the Proposed Work as agreed upon. If settlement has not been completed within thirty (30) calendar days from date of the work order / invoice, Hirth Plumbing & Heating Company retains the right to remove the materials and parts at the expense of the property owner where the Proposed Work was completed.

Standard Warranty

Hirth Plumbing & Heating Company does not warrant cabling of any drain lines.

Repairs Or Replacement Of Existing Piping And Apparatus Have A One Year Parts And Labor Warranty From Date Of Work Order / Invoice.

Installation Of New Fixtures, Faucets And Pumps Have A Two Year Parts And Labor Warranty From Date Of Work Order / Invoice.

In-Sink-Erator Garbage Disposers Have A Manufacture Warranty As Listed By Model.

New Water Heaters Have A Five Year Parts, Labor, Tank And Glass Lining Warranty.

The Standard Warranty Is Limited To Parts And Labor Provided And Installed By Hirth Plumbing & Heating Company and does not include existing parts, trim, apparatus or parts provided by others.

Statement Of Acceptance is a part of all work order / invoices

I, _____________________________________________________ as the owner of the property located at ____________________________________________ in the city of ___________________________ in the State of Illinois have read, understand and hereby authorize the Proposed Work and agree to the Terms and Conditions as stated on both sides of this form. I warrant that I am the owner of the property where the Proposed Work is to take place and grant Hirth Plumbing & Heating Company access to complete the Proposed Work. I further understand that cutting or removal of finished surfaces may be required to complete the Proposed Work and the cost to repair, replace or refinish these surfaces is NOT included in the amounts for the Proposed Work. I understand and agree that the existing piping, fixtures, or equipment with associated apparatus may no longer be serviceable or repairable and I agree to hold Hirth Plumbing & Heating Company, its owners, agents and employees blameless to any damages or destruction to these items in the course of completing the Proposed Work. I acknowledge and accept the schedule of fees and or interest charges for returned checks or credit card transactions used to pay for the Proposed Work including all costs associated with the collection of these fees. A deposit in the amount of $____________________ has been provided towards the completion of the Proposed Work.
